Orthodontic Treatment Combined with Orthognathic Surgery
Surgical orthodontic cases involve the repositioning of the upper and/or lower jaws (maxilla and mandible) by an oral surgeon working in collaboration with the orthodontist.
These cases arise when facial structural imbalance precludes successful orthodontic treatment using appliances such as braces and aligners.
Typical presentations would include upper jaw retrusion (recessive midface), lower jaw retrusion (small lower jaw relative to upper), lower jaw protrusion (large lower jaw relative to upper), or a combination of skeletal imbalances.
During treatment planning, the orthodontist, along with the oral surgeon and patient, determines the ultimate positions of the jaws and teeth that will result in correct dental occlusion, an attractive smile, and a balanced facial aesthetic.
When planning a surgical case, the orthodontist takes into account the patient’s overall facial structure and the impact on facial aesthetics of the positioning of the bones that will correspond to the planned tooth placement.
As treatment proceeds, the orthodontist makes pre-surgical changes to the teeth, positioning the upper and lower dental arches within their respective bones for proper occlusion to occur after the agreed-upon surgical procedure.
Post-surgical orthodontic treatment is limited to detailing of the occlusion to arrive at a stable, healthy bite and smile.
Dr. Wong’s Experience in Surgical Orthodontics
Although not a widely understood procedure, surgical orthodontic cases are not rare. Dr. Wong has extensive experience in such cases and refers to and works only with leading oral surgeons who regularly engage in collaborative orthodontic treatments.
Referrals to collaborating oral surgeons are based upon the procedure to be performed and the success and experience of the surgeon in such procedures. The choice of oral surgeon, however, is ultimately that of the patient, who must be completely comfortable with his or her team members.
